Dear New Patient,

Thank you for requesting an appointment with our practice.

Your first visit to our office generally requires 90-120 minutes of your time. This includes a detailed history of your illness, a physical examination by the doctor and possibly allergy testing. We commonly perform skin tests to determine exactly what in your environment is making you ill. Guided by discussion with you and by the findings of your physical exam, the doctor will determine which tests are needed in your individual case.

Many prescription and over the counter medications can interfere with our allergy evaluation. Certain Medications should be stopped prior to your visit. Antihistsmine/Decongestants (Allergra, Clarinex, Claritin, Zyrtec, Xyzal, Actifed, Benadryl, Dimetapp). Whether over the counter or prescription antihistamines should be stopped at least 48 hours prior to your visit. Atarax, vistaril, doxepin, or zyrtec are time release antihistamines and should be stopped 72 hours prior to your visit. Hismanal may need to be stopped for thirty days or longer.

Asthma Medications (Maxair, Ventolin, Theophylline) should be stopped 12 hours prior to your exam. If your health would severely suffer, then do not discontinue them. Theophylline preparations do not need to be stopped. Steroid tablets (Medrol, Prednisone, Hydrocortisone), or steroid inhalers (Vanceril, Beclovent, or Azmacort) should not be stopped.

If past experience suggests that any of the above changes will cause you to have problems particularly with development of asthma or hives, please contact us before you stop the medications. Please call if you are not sure how long you should be off your medications prior to your visit.

We would like your visit to be as pleasant as possible. Please remember to bring in your completed Patient Information Forms (pages 2-5), your insurance card, and your co-payment.
