Dear New Patient,

Thank you for requesting an appointment in our office.

The first visit to our office generally requires 90-120 minutes of your time. This includes a detailed history of your illness, a physical examination by the doctor and possibly allergy testing. If you have previous medical or allergy work-ups which may help in your treatment please bring them in. A list of the medications you’re taking is also helpful.

We commonly perform skin tests to determine exactly what in your environment is making you ill. Guided by discussion with you and by the findings of your physical exam, the doctor will determine which tests are needed in your individual case. All antihistamines should be stopped at least 48 hours prior to your visit.

We would like your visit to be as pleasant as possible. Please remember to bring in your completed Patient Information Forms (pages 2-5), your insurance card, and your co-payment. If you have any questions please call.
